Direct Primary Care Membership
We offer direct primary care starting at $70 per month: That means no insurance copays, no balance billing, more time with your provider, ability to email or text your provider, same day or next day appointments via telehealth. Services offered: Health Risk Assessment, Comprehensive Lab Screening*, Psychosocial Screening. Well visits, sick visits, Work and sports physicals, Blood pressure diagnosis and treatment, Diabetes diagnosis and treatment, Asthma treatment, Back, neck, knee pain treatment, Trigger Point Injections, Urinary tract infection testing and treatment, STD testing, Influenza, covid, and strep throat testing and treatment, Sinus infection, ear infection, Diabetes teaching and management, High cholesterol screening, Weight management, Hormones. Discounted cash prices for lab testing, In house blood draws for vitamin testing, hormones and more, Referral and coordination for specialists, FMLA paperwork and more. Membership includes one yearly comprehensive lab panel for insurance patients.
Patient is entitled to 12 office/telemedicine visits per 12 months for acute conditions, new diagnosis and follow up visits as deemed necessary by the provider.
Your membership includes one Lipo-Mino B vitamin shot per monthly paid membership. Members get 10% off Vitamin injection add ons, IV’s and IV add ons, toxins, (eg Botox, Jeuveau) dermal fillers, retail purchases.
